Abstract

Introduction: Cerebral venous thrombosis is a rare form of cerebrovascular disease caused by occlusion of the dural venous sinuses. Risk factors include the use of oral contraceptives and SARS-CoV-2 infection.

Objective: To report two cases of cerebral venous thrombosis associated with the onset of SARS-CoV-2 infection during the use of oral contraceptives. Clinical.

Case report: Case1 reported a 29-year-old female patient with a history of taking contraceptive pills and SARS-CoV-2 infection. She had headache in the left occipital region, which appeared slowly and progressively, which did not resolve with oral analgesics. The complementary examinations revealed positive D-dimer; skull tomography with turgid elongated hyperdense image in the projection of the left transverse and sigmoid sinus. Simple skull MRI with contrast and angio-resonance technique confirmed the diagnosis of cerebral venous thrombosis. Treatment with low molecular weight heparin started immediately and contraceptive pills were discontinued. The patient evolved satisfactorily with any other complications.

Case 2 reported a 20-year-old female patient with history of taking contraceptive pills and having had SARS-CoV2 infection, one month before the onset of symptoms. She began with right suboccipital headache with partial relief through the use of oral analgesics. The complementary examinations showed positive D-dimer; skull tomography with turgid elongated hyperdense image in the projection of the right transverse and sigmoid sinus. Simple skull MRI with contrast and angio-resonance technique that showed tortuosity of the venous system with signal defect. She underwent treatment with low molecular weight heparin and contraceptive pills were discontinued. The patient had a satisfactory clinical evolution.

Conclusions: The cases reported are examples of cerebral venous thrombosis associated with the use of oral contraceptives and COVID-19 infection, so it is vitally important to detect the disease in time when the typical clinical-imaging picture and these factors occur, along with associated risk, to promote timely diagnosis and treatment
